Weight Loss Medications and GERD

Ever had that sour taste unexpectedly creep up your throat? Acid reflux or GERD (Gastroesophageal Reflux Disease) can bring about uncomfortable symptoms like belching, a bitter mouth, heartburn or even a persistent sore throat or cough. Excess weight or obesity can exacerbate GERD due to increased abdominal pressure on the esophageal sphincter, allowing stomach acid to flow back up.

Weight loss is often the first step in managing GERD. However, many individuals undergoing weight management may be prescribed medications such as Zepbound, Mounjaro, Ozempic, or Wegovy from the GLP-1/GIP or GLP-1 class. These medications, while aiding weight loss, can paradoxically cause or worsen acid reflux due to their effects on the gut.

If you’re navigating GERD while on weight loss medication—or even if you’re not on medication, but struggling with heartburn—here are some strategies to consider:

Identify Trigger Foods: Keep a food diary to pinpoint and reduce intake of foods know to trigger reflux, such as spicy foods, fatty foods, caffeine, citrus fruits, chocolate, carbonated beverages, onions, garlic, alcohol and mint

Eat Smaller, More Frequent Meals: Weight loss medications often reduce appetite and portion sizes. Eating smaller meals more frequently can help prevent overeating which may trigger reflux

Time Your Meals: Have your larger meal earlier in the day, rather than at dinner or directly before bedtime

Stay Upright After Eating: Remain upright for several hours after meals to aid in digestion and reduce the likelihood of acid pushing back up into esophagus when you lie down to sleep

Wear Comfortable Clothing: Avoid tight fitting clothing that can increase abdominal pressure and exacerbate reflux

Sleeping Position: Elevated your upper body with pillows when sleeping to help prevent reflux during the night
